Below you will find all our forms required to begin services. We request these to be completed and in our possession at least 24 hours before your first appointment.  This allows therapists an opportunity to review your information and it will prepare you to the get the most out of your 90-minute intake session.  Failure to have your paperwork turned in before your appointment may result in having to reschedule.
